CANCELLATIONS/SUBSTITUTIONS

All cancellations must be made by or submitted in writing to [email protected] by June 24, 2024.  A $50 cancellation fee will be assessed unless a substitution is designated.   

Please be aware that by registering for this event, you consent to your voice, name, and/or likeness being used, without compensation, on Michigan Energy Providers Conference (MEPC) website/social media or the websites/social media of others in attendance, whether now known or hereafter devised, for eternity, and you release MEPC, its successors, assigns, and licensees from any liability whatsoever of any nature.

HEALTH ACKNOWLEDGEMENT

Those attending the MEPC must agree to the terms of this Assumption of Risk, Release of Liability, and Waiver of Claims (Waiver and Release).

MEPC has taken preventative measures to reduce the spread of communicable disease including, but not limited to COVID-19 (“Communicable Disease”) at its annual conference (“Event”).

However, I acknowledge that MEPC cannot guarantee that I will not be exposed to or contract a Communicable Disease at the Event. Further, due to the nature of the facilities, services and programs offered at this Event, I understand that attending the Event could increase my risk of contracting a Communicable Disease. 

ATTENDEE REQUIREMENTS

I agree to abide by all applicable COVID-19-related requirements, advisories, policies, procedures, and protocols of the resort and MEPC, as well as the CDC, state of Michigan, and any other governmental authority for the duration of my stay. 

If masks are required when the event convenes, I will comply with all guidelines. Failure to comply and abide by our rules could lead to expulsion without a refund. In consideration of being allowed to participate in the Event, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, acknowledge and agree as follows:

ASSUMPTION OF RISK

I have read and understood the above warning concerning COVID-19.  I understand my attendance is voluntary and hereby accept the risk of being exposed to, contracting, and/or spreading COVID-19 at the MEPC Conference.  

WAIVER OF LAWSUIT/LIABILITY

I hereby release and waive my right to bring a claim or lawsuit against the MEPC planning team members and its officers, directors, managers, officials, trustees, agents, employees, or other representatives for any liability in connection with exposure, infection, and/or spread of a Communicable Disease related to my attendance at the MEPC Conference.  I understand that this waiver means I give up my right to bring any claim based in negligence, including for personal injuries, death, disease or property losses, or any other loss, whether known or unknown, foreseen or unforeseen.

I have read, understand, and agree to the Attendee Requirements, Assumption of Risk, and Waiver of Liability and Assumption of Risk.  I am making a commitment to do my part to ensure a safe conference for all attendees.
